The Final Exam Proctor Request forms are different from the regular test/quiz request form.
The Final Exam Proctor Request (pdf) is also available in the office (green form).
All final exam request forms are due no later than Wednesday, April 24 by 5 p.m.
There will be no exceptions to this deadline due to the high number of exams that we are proctoring during exam week.
You will need to submit your Final Exam Schedule and Proctor Request Form
- Final Exam Schedule: A copy of the Final Exam schedule can be found via the Registrar's Office. You can also consult your course syllabus.
- Final Exam Proctor Request Form: This form is available on our web page as a word document and can be submitted in-person or via email to our email, disabilityresources@longwood.edu
